Cooper Lighting Solutions is repairing certain All Field and Prism RGBA fixtures manufactured between January 2016 and May 2019. Based on reports representing .001% of all affected lenses currently installed, lenses can loosen and separate from the fixture. Although the risk is low, Cooper Lighting Solutions wants to ensure that all fixtures are repaired.

Why are we taking this action?

Cooper Lighting Solutions has received a small number of reports that lenses have loosened and separated from fixtures at indoor facilities, which represent .001% of all affected lenses currently installed. There have been no reported issues involving Prism RGBA fixtures or All Field fixtures installed outdoors. There have been no reported injuries. Based on these reports, we have decided to take action to repair the fixtures in cooperation with the US Consumer Product Safety Commission.
